Minors visiting without an adult must have a waiver form submitted by a parent or guardian before their visit. If arriving without a signed waiver, they (you) must contact an adult to provide the required signature.

If you are 18 or older, you don’t need to sign the waiver form in advance—you can simply complete it when you visit the lounge.
